SUBJECT: Change Order Number 003 for the Mechanical Infrastructure System Upgrade and
Expansion: Main Terminal AHU-8 Acoustic Upgrades and Miscellaneous HVAC Modifications
project, Contract Number: MC-0314919
ACTION REQUESTED: This memorandum requests Commission approval for the Chief
Executive Officer to issue Change Order Number 003 for the Main Terminal AHU-8 Acoustic
Upgrades and Miscellaneous HVAC Modifications project to add the additional time required
(89) days to complete the project.
SYNOPSIS: Operational issues with Elevator 3F impacted the contractor’s ability to mobilize
equipment and materials to and from the Terminal work area during the course of the project,
which delayed project completion. A change order is required to extend the contract completion
date for this Port-owned delay.
CHANGE ORDER DESCRIPTION AND JUSTIFICATION: The following information
relates to the pending change order scope and cost:
Change Order No. 003
Scope of work: The current Contract completion date is August 9, 2008. Operational
issues with Elevator 3F impacted this contract throughout its implementation. Following
Beneficial Occupancy for this project, Elevator 3F was out of service for several weeks.
This affected the contractor’s ability to mobilize equipment in and out of the penthouse
area and complete the removal of glue stains left from the carpet removal in the old
Police locker rooms. The project was completed November 05, 2008. A no cost time
extension of 89 days is requested to reconcile the contract completion date.
Justification: The contractor was not determined to be responsible for the delay.
Consequence of not proceeding: The project documentation would be incomplete to
provide justification for the contractor exceeding the contract completion date.
